Should You Worry About That Grinding Sound in Your Neck? Crepitus Explained

You turn your head to check your blind spot, and there it is again — that gravelly crunch, like sand grinding between the bones of your neck. It doesn’t exactly hurt, but it doesn’t sound healthy either. So you do what most people do: you turn your head again to see if it happens twice, then head to the internet at midnight to find out if your spine is falling apart.

Here’s the short answer up front: neck sounds are extremely common, and most of the time they’re harmless. But “most of the time” isn’t “always,” so let’s walk through what that sound actually is, when it matters, and what you can do about it.

What Crepitus Actually Is

Crepitus is the medical term for the popping, cracking, grinding, or crunching sounds a joint makes when it moves. In the neck, those sounds typically come from a few different sources:

  • Gas bubbles collapsing in the joint fluid. Your facet joints are lubricated by synovial fluid. Pressure changes can form and collapse tiny gas bubbles — the same mechanism behind a knuckle crack. Sudden, painless pops usually belong in this category.
  • Tendons and ligaments snapping over bone. As tissues move across bony ridges, they can flick or snap — more of a click than a crunch, often the same spot every time.
  • Roughened joint surfaces gliding on each other. As cartilage wears with age, joint surfaces lose their glass-smooth finish, and movement can produce that sandy, grinding texture — the classic “gravel neck.”

Why Grinding Gets Louder With Age (And Desk Jobs)

Some cartilage wear is a normal part of living in a human body — by middle age, most people’s cervical spines show age-related changes on imaging, including plenty of people with zero pain. Grinding sounds often track with those normal changes, which is why crepitus tends to get more noticeable through your 40s, 50s, and beyond.

Posture accelerates the process. Hours of forward-head position — the desk-and-phone pattern — concentrates load on joints that were designed to share it. Joints under uneven load stiffen, tissue around them tightens, and movement gets noisier.
